It was designed as a place for patients that regularly visit Facebook to discover trusted health information, wellbeing tips and local events to support healthier lifestyles.
As a patient, it is all about being aware of services that can support and enable a healthier lifestyle in the community.

Social prescriber and care coordinator support
Our Social Prescriber, Lisa, and our Care Coordinator, Natasha, are here to support our patients.
They both have slightly different roles but work together to support our patients.
They will both speak with patients that need:
- Support at home, care needs assessments, social care referrals
- Home visits to support care needs
- Safeguarding issues, ambulance notifications
- OT referrals for equipment at home
- Mobility assessments
- Ordering care technology, falls alarms etc
- Welfare calls after hospital discharges
- Frailty assessments
- Point of contact for patients to navigate health and social care system
- Supporting carers and families
- Signposting patients and families to community services
